• When filling out the form, make sure it is legible.
  
• Under the section “Legal Name of Entity,” use your state abbreviation, dash, Chapter designation letter, as provided on your Chapter Charter from the Home Office (Example AZ-S.)
  
• There is no need to put aliases or nick names.
  
• Once you have received your EIN, you must send it to the appointing Officer and that Officer must forward the EIN to the Home Office.

Upon the change of Directors for a Region, District, or Chapter, you must fill out the 8822 form available at IRS.gov.

• The EIN number must follow the Chapter with only the name and address changing to the newly appointed CD.
  
• For GWRRA’s records, it has to remain in the name of the CD not other staff.
  
• When filling out the 8822 form, you must make sure it is legible.
  
• Where the signature space is provided, please print your name under the signature.
  
• The form does not call for this, but this will assist the IRS in being able to read your name.
